    Disabled Person Parking Permit 83039 properly filled out and signed by the doctor of the applicant. (required)

    Method of payment: Cash, Check or Debit Card.  If paying is other than cash there’s an additional 2% processing fee.  Debit cards must have a chip and not be Credit Union cards.
